This survey should be completed by the person who serves as director or coordinator of student disability services at your institution. All individual answers to the following questions will be kept strictly confidential; only statistical results will be reported.

The purpose of this survey is to compile information about differences and similarities among disability support offices and staffing across New York State institutions of higher education. It has been developed by a group of disability service providers representing both public and private institutions. This information will be useful as we explore avenues to greater salary and program support and as we expand professional development opportunities for disability service providers statewide.
